There is no other form outside this portal.<\/p>\n\n\n\n<p>The applicant may choose between an application submitted using the&nbsp;<strong>national electronic identification<\/strong>&nbsp;and authentication scheme (hereinafter referred to as \u2018NIA\u2019) and a standard electronic application. If the application is submitted using the NIA form, the application is delivered to the Civil Aviation Authority of the Czech Republic (hereinafter referred to as \u2018the CAA\u2019) automatically after the application has been completed and confirmed as sent, so it is a faster way of submitting the application.<br><br>If the application is completed using a&nbsp;<strong>standard electronic form<\/strong>, the form must be printed, signed and delivered to the CAA together with all attachments by post, in person or by Czech data box. Contact details of the CAA can be found&nbsp;<a href=\"https:\/\/www.caa.gov.cz\/en\/contacts\/\">here<\/a>. In this case, the completing of electronic form is not sufficient \u2013&nbsp;<strong>proceedings are not initiated until the CAA receives the signed application.<\/strong><\/p>\n\n\n\n<p>Please note that the signed application must be delivered to the CAA CZ within 3 months.
