Forms submitted more than 3 months in advance will\nnot be accepted.<\/p>\n\n\n\n<!--more-->\n\n\n\n<p>All submissions must be filled out electronically.\nFollow the links below in order to fill out the forms. The forms must be\nsubsequently delivered to the CAA.<\/p>\n\n\n\n<p><strong>Electronic form for new\nBackground Check Certificate<\/strong> (available in Czech and English version):<\/p>\n\n\n\n<p><a href=\"https:\/\/ros.caa.cz\/\">https:\/\/ros.caa.cz\/<\/a><\/p>\n\n\n\n<p><strong>Electronic form for the Duplicate of the Background\nCheck Certificate<\/strong> replacing the previously issued Certificate in\ncase of loss, theft or damage(available in Czech and English version):<\/p>\n\n\n\n<p><a href=\"https:\/\/ros.caa.cz\/duplikat\">https:\/\/ros.caa.cz\/duplikat<\/a><\/p>\n\n\n\n<p>Sample forms are available&nbsp;<a href=\"https:\/\/www.caa.gov.cz\/dokumenty\/formulare\/formulare-sekce-spravni-a-bezpecnostni\/zadost-o-overeni-spolehlivosti\/\" target=\"_blank\" rel=\"noreferrer noopener\" aria-label=\"here (opens in a new tab)\">here<\/a>.<\/p>\n\n\n\n<p><strong>From 1\/1\/2020 only the forms filled out electronically are accepted. Currently valid form (which might be filled out in hand) was accepted alongside the electronic version only until 31\/12\/2019. <\/strong><\/p>\n\n\n\n<p><strong>In order to submit your application you will need:<\/strong><\/p>\n\n\n\n<p>1. The FORM available on the links above (choose your language preference on the web page)<\/p>\n\n\n\n<p>2. Print out and sign the form  or use a data box of a natural person to submit your application.<\/p>\n\n\n\n<p>3. It may become necessary to submit following attachments:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>extract from the Criminal Records of a foreign country;<\/li><li>ID in case you are a foreigner who is not recorded in the Population Records of the Czech Republic.<\/li><\/ul>\n\n\n\n<p>You may find further\ninstruction regarding the attachments in the e-mail that you receive after you\nfill out your application online.<\/p>\n\n\n\n<p>4. The application together with all the required attachments needs to be submitted to the CAA in one of the following ways:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li><strong>in person<\/strong> to the filing department of the CAA, with its seat at K leti\u0161ti 1149\/23, Prague&nbsp;6, 160 08;<\/li><li>via a <strong>data box<\/strong> of a natural person* (data box identifier: v8gaaz5);<\/li><li>via <strong>postal services <\/strong>to the filing department of the CAA, with its seat at K leti\u0161ti 1149\/23, Prague&nbsp;6, 160 08 (by way of exception, this does not apply to foreigners who do not have an officially authenticated copy of their identity card \u2013 they are obliged to submit their application in person and present a valid ID and an extract from the Criminal Records from the country of their citizenship).<\/li><\/ul>\n\n\n\n<pre class=\"wp-block-preformatted\">* The applicant must have his data box set up as a natural person. Data box which was set up by the applicant\u00b4s employer does not meet this requirement. All attachments delivered electronically are subject to authenticated conversion of the documents.<\/pre>\n\n\n\n<hr class=\"wp-block-separator\"\/>\n\n\n\n<p>Kindly note that the holders\nare obliged to return the Certificate which expired no more than 5 days from\nthe date it expired.
